Before you arrive
The hospital sends your records. We confirm insurance and call you or your family to answer questions.
First day
Nursing and therapy assess you, and we set goals with you and your family, including the date you're aiming to go home.
Each week
A care conference reviews progress. Families can join in person or by phone.
Going home
We arrange equipment, home health and follow-up appointments, and do a home-safety review before you leave.
